Your community-based approvals have been interrupted (you are still eligible, no screening required) and need to be restarted – an immediate visit with your Service FacilitatorA Medicaid-enrolled provider who supports eligible people, and sometimes their families, in properly using consumer-directed services. More upon discharge is highly recommended. Please keep us notified.
